St. Joseph Mercy Hospital offers up to twelve Transitional first year positions for physicians who plan to enter a specialty that requires a broad-based, clinical first graduate year or for those physicians who are unsure of their future medical goals. The Transitional Program's national reputation attracts highly qualified applicants from across the country. Appointments are made through the National Resident Matching Program and applications are accepted only through ERAS - Electronic Residency Application Service.

The Transitional program has as its major sponsor the Department of Internal Medicine with support sponsorship coming from the Department of General Surgery.

The program stresses training in Internal Medicine, providing a five-month experience in Internal Medicine including one month in the Intensive Care Unit, and a one-month outpatient general medicine experience. Transitional interns also rotate through one month of Emergency Medicine and one month of specialty Surgery. Transitional house officers have 3.5 months to devote to the electives of their choice, depending on their goals and future residency. Elective choices include a high variety of internal medicine and surgical subspecialties, dermatology, ophthalmology, anesthesia, PM&R, Radiology, pathology, radiation oncology and interventional.
