Application for rotation(s) must be returned to the Graduate Medical Education (GME) office at least one month prior to the start of the rotation. The following must be submitted:

( ) Application ( ) Confidentiality Agreement ( ) Copy of your institution’s employment contract or signed offer letter ( ) Curriculum Vitae (CV) which includes your current year in your residency or fellowship program ( ) Copy of your medical school diploma ( ) Accreditation letter from ACGME or AOA showing that your program is currently accredited ( ) Copy of Michigan Medical License ( ) Copy of Michigan Controlled Substance License ( ) Copy of Federal DEA License, if applicable ( ) Copy of ECFMG Certification, if applicable ( ) Rotation schedule indicating location of rotation is at SJMH ( ) Completed background check form
We will contact you once your application has been approved or rejected. NOTE: Applicants are not eligible to practice at SJMHS until the registration process is completed.
