Exercise for Weight Loss

ShapeDown Weight Management for Children and Teens

Chronic Disease Management
Center for Diabetes

Learn to manage Type I, Type 2, or Pre-Diabetes so you can enjoy a full and active life! Through a combination of individual and group education, our Certified Diabetes Educators will teach you about diabetes and show you how to care for your health. Family members and other support people are encouraged to attend, to help you maintain healthy diabetes self-care practices. Physician referral required; often covered by insurance. - Understand diabetes and how it affects your body
- See how nutrition, exercise and medication can help control blood sugar
- Learn how to check your blood sugar
- Plan nutritious meals

PATH (Personal Action Toward Health) Workshops Take The PATH to Better Health! PATH is a fun, interactive workshop that provides skills and tools needed for people living with ongoing health problems to improve their health and manage their symptoms. As a result, participants are better equipped to face the daily challenges of living with a chronic condition. Two trained leaders conduct the 6-week series (one or both may have a lifelong health condition).
This program will help you:
- Deal with the challenges of not feeling well
- Talk to healthcare workers and family members
- Overcome stress and relax
- Increase your energy
- Handle everyday activities more easily
- Stay independent
Nutrition Education
Develop healthier eating habits to lower you blood pressure or cholesterol, control your diabetes, lose weight, or manage a health condition affected by nutrition. In group classes or one-on-one with a registered dietitian:
- Learn how the foods you eat affect your health
- Learn how to make healthy food choices in a variety of settings
- Gain skills for cooking healthy meals
